The antifungal medication terbinafine is the active ingredient of Lamisil cream, a topical prescription drug for fungal skin infections. Terbinafine works by interfering with a fungal cell’s ability to maintain its cell membrane, the protective membrane that surrounds the cell and holds it together. With repeated exposure to the drug, the fungus is unable to grow; it eventually dies and the infection is cured.

Currently, Lamisil is probably the best FDA-approved drug for the treatment of fungal infection. In addition Lamisil tablets are available which are recommended for treatment of fungal nail infections which are difficult to cure with topical preparations. Studies have shown a better treatment success rate with terbinafine than with other available prescription drugs such as griseofulvin and itraconazole.

Both Lamisil cream and Lamisil tablets are used daily and usually need to be continued for a number of weeks to completely wipe out the fungus. Skin infections usually take from one to four weeks to resolve. Nail infections will often clear up in six weeks, while toe nail infections take four months or more to cure. It’s not uncommon, however, for much longer periods of treatment to be necessary, especially in the case of toe nail fungus infection. Complete replacement of the disfigured nail takes many months, even if the fungus is dead, because nails grow very slowly.

Systemic antifungal medications have had a reputation for causing unpleasant side effects, most notably liver damage, and although clinical studies of Lamisil tablets have indicated that these side effects are relatively rare, and usually quite mild, they are still a concern. Patients taking Lamisil tablets should watch for signs of liver toxicity, such as fatigue, nausea, vomiting, yellowing of the eyes and skin, dark urine, or light colored stools. Any other unexpected symptom that appears during the course of treatment should also be reported to a doctor. Concerns are not as great with Lamisil cream because the cream is only applied locally and relatively little gets into the bloodstream.

Another concern with Lamisil tablets is the possibility of drug interactions, and there is a long list of drugs that should not be used together with Lamisil tablets, or that should be used only with caution. The Lamisil dose may need to be adjusted to prevent adverse drug interactions and side effects. Anyone taking any other prescription medication should check with their doctor and their pharmacist before taking Lamisil. Again, Lamisil cream does not carry the same concerns; however, it is always worth to ask an appropriate medical professional before mixing medications.

Also known as onychomycosis and tinea unguium, nail fungal infections are the most common diseases of the nails, making up about 50% of nail abnormalities. Both fingernails and toenails are susceptible to the infection, which usually manifests as discoloration and thickening of the nail and crumbling edges. The condition most commonly occurs in toenails.

Between six and eight percent of the adult population suffers from nail fungus infection.

Who gets nail fungus infections?

Nail fungus infections are more common in men than women and in the elderly than the young. Additional traits or factors that raise one’s risk of nail fungal infection include the following:

Diminished blood circulation

Slow growing nails

A family history of fungal infection (genetics)

Heavy perspiration

Humid or moist work environment

Wearing socks and shoes that prevent ventilation

Walking barefoot in damp public places (swimming pools, gyms and shower rooms)

Previous injury or infection to the skin or nail

Diabetes, AIDS, circulation problems, a weakened immune system

Tight footwear with crowding of toes

Exercise that causes repeated minor trauma to the hyponychium (where the finger tip attaches to the nail)

What causes nail fungus infections?

Nail fungal infections are caused by microscopic organisms called fungi that do not require sunlight to survive. Most commonly, a group of fungi called dermatophytes (such as Candida) is responsible for nail fungal infections. However, some yeasts and molds also cause these infections.

Though Trichophyton rubrum is the most common dermatophyte that causes nail fungal infections, Trichophyton interdigitale, Epidermophyton floccosum, Trichophyton violaceum, Microsporum gypseum, Trichophyton tonsurans, and Trichophyton soudanense may also cause the infections. Common mold causes include Neoscytalidium, Scopulariopsis, and Aspergillus.

Pathogens that cause nail fungus infection usually enter the skin through tiny cuts or small separations between the nail and nail bed. The fungi grow when the nail provides a suitably warm and moist environment.

What are the symptoms of nail fungus infections?

Nails that are infected with fungus typically are thickened, brittle, crumbly, ragged, distorted, dull, and darker or yellowish in color. A patient may also experience onycholysis, where infected nails separate from the nail bed. Sometimes, nail fungal infections result in pain in the toes or fingertips, and they may even emit a slight foul odor.

Another symptom associated with nail fungus infections are fungus-free skin lesions called dermatophytids. These may be rashes or itchiness in an area of the body that is not infected with the fungus – much like an allergic reaction.

How are nail fungus infections diagnosed?

In order to diagnose nail fungus infections, a doctor will usually examine debris that is scraped from underneath the nail. The nail scrapings will be used in tests such as a potassium hydroxide (KOH) smear or a fungal culture. The KOH test can be quickly performed, while the fungal culture can take weeks.

Physicians must be careful when diagnosing fungal infections of the nail because several other conditions can result in similar symptoms. These include psoriasis, lichen planus, contact dermatitis, trauma, nail bed tumor, eczema, and yellow nail syndrome.

What nail fungus treatments are there?

Treating nail fungus infections can be a long and expensive process. There are oral antifungal medications, topical ointments, and alternative therapies. Over-the-counter creams and ointments are available, but they have not proved very effective.

Oral medications for nail fungus infection include Itraconazole (Sporanox), Fluconazole (Diflucan), and Terbinafine (Lamisil), which typically take up to four months before fully replacing the infected nail with uninfected nail.

Topical nail fungus treatments include antifungal lacquer or nail polish such as ciclopirox (Penlac) in addition to other creams. Use of topical remedies can clear nail fungal infections, but often does not completely cure the infection.

In some extreme cases, a physician will opt to remove the entire nail.

Alternative medicines used to treat nail fungal infections include Australian tea tree oil and grapefruit seed extract. However, there is no scientific evidence supporting the use of these products.

How can nail fungus infections be prevented?

Preventing nail fungus infections requires proper hand and foot hygiene. Some suggestions include:

Keeping nails short, dry, and clean

Wearing socks that breathe, usually synthetic

Using antifungal sprays or powders

Wearing rubber gloves to avoid overexposure to water

Refraining from picking or biting nails

Wearing shoes or sandals in public places and pools

Ensuring that your manicure or pedicure salon properly sterilizes tools

Stopping use of nail polish and artificial nails

Washing hands after touching infected nails

Avoiding sharing shoes and socks

Lamisil (terbinafine) for Onychomycosis — Toenail: I was using Lamisil for almost 3 months. Developed muscle pain and doctor pulled me off. Other than that toes look perfect after just three months. I spent 35 years curing it myself with no significant results. By summer the wife wants me to wear sandals, something I haven’t done since I was 17. Big toes are about 75% back others are about 90%. Looking forward to barefoot sand walks.

Lamisil (terbinafine) for Onychomycosis — Toenail: My physician prescribed Lamisil for me in October 2008. I took it for three months and had 2 liver panels done during that time. I was dizzy and mildly nauseated the first few days and had diarreah. It has taken 10 months for the brand new nails to grow out but I can bare my toes again. It also treats athlete’s feet. Now my feet are baby smooth and free of cracks. I would definetely recommend this product. This is the best my feet have looked ever.

Lamisil (terbinafine) for Onychomycosis — Toenail: Hideous fungus under all 10 toenails since age 12, I’m 52 now. My life is changed, since in adolescence and even until today, I have hidden my toes and suffered ridicule and embarrassing questions. Been taking the drug for 4 mths as I write this. It’s a miracle. Big toenails have cleared nicely to 50% and all other 8 nails are at 95% or better. Taste impacted minimally(temporary). Liver function normal.

Lamisil (terbinafine) for Tinea Capitis: I’ve been taking the Terbinafine 250mg tab once daily for just over a month now and haven’t had any side effects that is noticeable . My toenails have cleared up and I still have 60 more days of treatment. if you take this medication as directed, with a full glass of water and do not drink alcohol, you’ll stand a much better chance of complete success. This is the first treatment I have tried since they took the Lamisil Cream off the market that has worked.

Lamisil (terbinafine) for Tinea Capitis: I have used Lamisil before and was very happy with the results. After the full course of medication my toenails looked greatly better. I had no side effects and I also have a seizure disorder, it worked great for me. My brother took it for the same reason and he had no side effects either. It worked for him as well. It was expensive but worth the results.
